Dear Kim
I refer to your medical results received by the Health Operations Centre
(HOC) for your Australian visa application. The HOC requires further
information before we can determine whether you meet the health criteria
for entry to Australia.
You are required to provide:
Medical examination (Form 26) completed by a recognised Panel Doctor.
Pathology report for HIVas you are over 15 years of age.
Please advise the Panel Doctor to forward the report, along with a copy of
this mail, to:
Health Operations Centre
Level 3, 26 Lee Street
Sydney NSW Australia 2000
A list of Panel Doctors recognised by DIAC can be found at this website:
http://www.immi.gov.au/
Processing requirements provide a time limit of 28 days (plus 21 days to
allow for postage) for the report to be forwarded to the HOC. If your
health assessment has not been finalised by this time, your case-officer
may proceed to decide your application on the basis of the available
information.
Please contact the HOC immediately by return email if you are unable to
meet the requested timeframe.
Yours sincerely
